Program Description

Event Details

The Dunedin Public Library is now a passport acceptance office. We can help you with your new passport needs, along with taking your photo for the application.

The Library can process up to two applications and two photos in your 30-minute appointment.

You are eligible to schedule an appointment with us if at least one of the following conditions are true:

  • You are applying for your first U.S. passport.
  • You are under the age of 16.
  • Your previous U.S. passport:
    • Was issued when you were under age 16.
    • Was issued more than 15 years ago.

If none of the above statements apply, you may be eligible to renew your passport by mail using Form DS-82.

If one of the above conditions are true, please fill out the Form DS-11 and continue to making an appointment.

For the most up to date information visit the Department of State website.

Appointment Reminders

Please note that you will be allotted the 30 minutes scheduled for your appointment to meet with an agent. If you do not have the correctly completed forms and other required documents with you at the beginning of your appointment, you will be asked to reschedule.

Please arrive on time to your appointment. Appointments for one person will be canceled if not claimed with 15 minutes of start time. Appointments for two people will be canceled if not claimed with 5 minutes of start time.

Please bring the following items to your appointment:

1. Completed DS-11 Application for a U.S. Passport: https://pptform.state.gov/. The library also provides blank forms that you can complete prior to your scheduled appointment time. Do not sign the application until instructed to do so by the acceptance agent.

2. Proof of citizenship and photocopy (black and white photocopy only).

3. Proof of identity and photocopy.

4. Processing & execution fees. You will need two checks or money orders per application, one for the U.S. Department of State processing fee and the second for the Dunedin Public Library execution fee.

All children under age 16 must apply for a passport in person with two parents or guardians using Form DS-11. If both parents/guardians cannot appear in person, additional documentation will be required. Visit https://travel.state.gov/content/travel/en/passports/need-passport/under-16.html for forms and information.

Renewals

If you already have a passport, you may be eligible to renew by mail. Passports issued within the last 15 years are generally eligible for renewal if still in your possession. Visit https://travel.state.gov/passports for more information.
